Steps to Apply

1. Complete the Privacy Policy and Malpractice Application form and download
   it to your printer.
2. Fax or e-mail the completed application with your signature and date.

3. Pre-Authorized Payment Plan (3% service fee): If you have chosen
   the preauthorized monthly payment plan option please also fax/email  a “void”
   cheque from your bank account.
   
4. Axa Canada requires original forms for legal necessity. Forward by mail all
   of the original faxed and emailed forms to:
   
   Walter Roberts Insurance Brokers Inc.,
   110 West Beaver Creek Road, Suite 22,
   Richmond Hill,
   Ontario, L4B 1J9.

Coverage Effective Date
Underwriting of your application commences the day of faxed receipt. In most
cases coverage may commence the day of receiving your application… of course…
acceptance of your policy is based upon a favorable underwriting review.
